Tayside Complete Health view the relationship between the clinic and its service users as a partnership for the delivery of high quality care. These terms and conditions form the basis of our contract with you
We aim to:
• Provide facilities accredited to undertake the range of tests and procedures offered to national standards.
• Maintain necessary registrations and specific licenses to operate.
• Ensure appropriate levels of skilled staff to meet clinical and other service needs of patients.
• Provide suitable levels of clinical supplies and support services to ensure maintenance of a safe environment for you.
• Maintain and develop risk management, quality assurance and training programs to ensure consistently high service standards.
Payment Terms We offer a range of payment options including self-pay, personal finance, and insurance options. We are recognised by most major insurers.
Self-pay:
• Guide Information of current fees are provided at time of booking your appointment. Tayside Complete Health reserves the right to amend prices as required
• Your will be required to pay in full at time of booking your appointment. You can pay by credit or debit card, we accept most major credit cards.
Personal Finance: • For more complex cases we offer personal finance through a trusted partner.
Insurance: • We are recognised by most major health care insurance companies. We will require an authorisation code from you at the time of booking an appointment with you. It is your responsibility to seek this from your insurer.
What’s Included • The consultation price will cover the consultation element only. Should you require further tests or treatment there will be additional costs associated with this. You are under no obligation to proceed and we will provide a quotation at every step (this is available in writing on request). • Some procedures (such as biopsies) also require a pathology cost - these will be in addition to the procedure cost but will be identified in advance. You have the right to accept or decline at every stage.

Valuables
• We cannot accept any loss or damage of any personal effects brought in by you. You are therefore requested to ensure that no valuable items are brought into the clinic. Personal Data • Tayside Complete Health treats the security of your personal data with the utmost importance. We will only process your data as is necessary for the provision or our health care services.
